This is a two-line segment from a hospitalization report. Maybe there is a punctuation error, maybe there is a word missing or maybe I am just tired today. The lines read:
בתשובה פטולוגית - בקפל מפלקסורה ימינית - רירית קולון.
בפוליפ בגובה 40 ס"מ -
poorly mod diff ADENOCA
המגיע לשולי הבריכה ללא
vascular invasion
If the punctuation is in the right place, it looks like it says "in the right flexure - colon mucosa in the polyp at a height of 40 cm.... etc. But that doesn't make sense to me.
Can anybody help with this one?

PMDA was found in a colonic polyp removed from a fold in the right flexure, at a height of 40 cm [from the anus, if removed by colonoscopy]. the process does not go beyond [or = invade] the incision margins [my guess of the spelling mistake] and there is no vascular invasion.

Basically you are correct. The passage is describing exactly where this polyp containing a poorly to moderately differentiated non-invasive adenocarcinoma was found.
