If you are subject to a child support order or wage withholding, you are required to provide a copy of the order(s) to your employer. This statute applies to any employee who is hired, rehired, or is returning to work from an unpaid leave of absence.

Wage Deduction Authorization
Direct Deposit Authorization Form
Federal Form W-4
State A4 Withholding Form

In accordance with Arizona Revised Statute #23-722.02 all employers are required to ask each new employee if they are subject to any child support orders or wage withholdings.
