: I assume it should be kept in the patient chart (the ‘status’ (chart) differs from the ‘dossier’ (file)) for immediate availability. It will be removed when the trial is completed.// See D-box for 'status'. Thank you in advance for considering.

Thanks. It's dossier (a file), as per van Dale.

: The word "status" in Dutch actually means case history or anamnesis. It can be either a chart or a dossier. A chart is usually used to record the current treatment and the history would usually be in the file. See van Dale on Status.

Een 'status' in ieder geval niet, dat is een map waarin je de gegevens van de opgenomen patiënt bijhoudt. See for 'patient's chart', for instance, the description in the discussion box.

retain in patient's file
I think this would be the way it would be said in the UK. Of course, it is just another alternative for the answers already given but I feel "retain" is more likely to be used in this context than "enclose" and it is just a bit more formal than "keep" ;-)

Physicians, too, have several concerns regarding the informed consent process. “What we hear from physicians is they want access to the forms. Physicians want the information at their fingertips without having to ask other people to go around and look for it. And they want to know before they start a procedure if this information [a signed consent form] is in the [patient’s] chart or not,” says Sam Rubinsztain, vice president of product development at gMED (General Medical Applications).

Mag ik vragen waarom je hebt besloten om voor 'file' te kiezen?
bewaren in status (dat moet een soort map zijn)
Ik weet echt wel wat een status is, ik heb ze zelf ingevuld.

charting
[chahrt´ing]
the keeping of a clinical record of the important facts about a patient and the progress of his or her illness. The patient's chart most often contains the history; laboratory reports; list of medications; results of physical examinations, consultations, and special diagnostic tests; treatments of the health care team; any problems; and the patient's response to interventions and treatment.
