Okay, the patient underwent laparoscopy, cyst removal in a same-day surgery setting...

Name of physician? It's a very long shot, but https://www.anregiomed.de/fileadmin/Bilder_und_Dateien /02_M... lists a Carlos Andres Tapia Figueroa as "Leitender Oberarzt". That said, "von Tapia als 1 Punkt geändert" sounds like an internal note, and "als ... geändert" is quite a strange wording in my view. What does the report say after your text fragment?

agree Björn Vrooman : I'll close this Q because Steffen did supply the right answer. As for "als 1 Punkt geändert," could be "geändert..." or "...zusammengeführt unter diesem (Listen-)Punkt." Like "als" vs. "wie," pretty colloquial.
