It had been talked about for a few weeks, but it all started for me in Dubai at the Med Lab
conference. It was February 2020 when the Chinese colleagues were suddenly ordered to
run away. From that moment the lockdown began all over the world, in a surreal atmosphere
of a Hollywood disaster movie. Very intense months began, with the challenge of making
progress on the relaunch of DIESSE just six months after its takeover in the middle of the
pandemic. The fear and the courage we and our colleagues felt was mixed with the energy
of knowing we had to play our part in the battle against the pandemic. My commitment as
president of Confindustria medical devices was inextricably linked to all of this: to help our
country, desperately reaching for the many medical devices missing in hospitals. It became
evident to all that there weren’t enough medical devices produced in Italy. In fact, white
economy is not only a moral duty, but also a strategic value. As I have said before, DIESSE,
with its short supply chain, is an example for everyone.

In the context of emerging viruses we have had the opportunity to get to know and develop a
very constructive relationship with DIESSE. This relationship can be traced back to the
Ebola epidemic, when DIESSE provided us with support, consulting, and help in the
development of a prototype ELISA test for the measurement of immunoglobulins against
various strains of Ebola virus. This repeatable and simple test was used on the ground, that
is, in Africa, in Sierra Leone, for the determination of the seroprevalence in the population
that had been the protagonist and unfortunately the victim of the epidemic that devastated
West Africa from 2013 to 2016. On the strength of this positive experience we have been
keeping productive and collaborative relationships with DIESSE: lately we have developed
one of the first serological tests for the measurement of IgG, IgM, IgA against Sars
coronavirus for which the whole virion was used as antigen. The virus we isolated at the
Spallanzani was exactly the strain of the first two Chinese tourists, made available through
the platform EVA G. The laboratory I direct at Spallanzani has conducted the entire
validation phase of this test and I must say that it is one of the first that was developed as
part of the response to the pandemic. Among those available at the time, it was the one that
certainly had the strongest correlation with the neutralizing ability of the sera against the virus
infectivity. In recent years we have gotten to appreciate the pragmatic, result-oriented and
constructive spirit of DIESSE, obviously combined with its great professionalism, flexibility,
and the ability to change quickly to meet practical and logistical needs.

It was March 2020 when the virus arrived at our DIESSE L3 labs. We had very little time to
perfect the process, then grow the virus in cultured cells, amplify it and finalize an effective
inactivation process, one that was later patented. This was done to obtain an antigen that
could be used in our diagnostic kits. That was at the start of the pandemic and the world was
facing the largest health emergency of modern times. At that time the virus was frightening.
And the fear was there for me and my team as well. When I go back to those days I almost
feel like I was dreaming. Because we really worked hard, day and night; we left home very
early in the morning and came back home very late in the evening. I remember the silence
and the empty streets in those days. For us it was a huge challenge against time and against
our fears, but it left us with the awareness of having made a little contribution to a great
battle.

Let me answer by presenting a considerable commitment that DIESSE has decided to
undertake. The commitment is to associate this activity aimed at facing the pandemic with a
specific brand. We have first of all inaugurated a logo called “DIESSE 4 covid 19”. DIESSE 4
covid 19 testifies to what every brand wants to make: a promise.
The first step of DIESSE 4 Covid 19 has been to make these first three tests available: the
IgG, IgA, IgM monitoring for SARS-CoV-2 with the Enzywell technology, available on
microplates for tools that at the beginning of the pandemic were the first defensive weapon.
Then, as the impact of the pandemic increased, we felt the need to apply this technology to
our other tool called Chorus, an Elisa system again. Therefore, thanks to Chorus we were
able to offer the market, and our first interlocutors, the first three tests we launched.
Afterwards, we launched a screening test. We then followed these up with a specific launch
for the determination of the antigen, to detect the presence or absence of infection, no longer
in serum, but in a nasal swab. Finally we got to a test that is perhaps the most popular test
today, certainly something we are really proud of. This is the very successful test for the
detection of the antibody, NEUTRALIZING AB. It quantitatively determines the reaction of
our immune system to having contracted an infection and, even more important, to the effect
of a vaccine. We have recently launched tests that will be performed in pharmacies, a line
that is called I-Rapid, as we want to offer quantitative monitoring of the presence of
antibodies to SARS-CoV-2.

In the meantime, vaccines arrived and it was immediately clear that, especially for
Messenger RNA vaccines, it was necessary to develop diagnostic tests able to verify that
the response of the humoral immune system was actually valid and effective, that it could
last for a long time and could be both a protection tool and an indicator of the ability to block
the virus and its most serious effects. Therefore, a neutralizing test was developed according
to the World Health Organization standards-the WHO preparation for the Chorus platform-
that was tested on the main variants of the virus known so far, and which still uses the
inactivated virus as a matrix.

The use of native virus has led us to the development of extremely sensitive kits, as in our
antigen all viral proteins are present and viral proteins keep their own structure, their own
folding, so they are the most suitable antigen for the detection of antibodies. This antigen
has been crucial to the development of all our subsequent products, especially the test for
the detection of neutralizing antibodies against the Spike protein. That test is based on the
use of our native antigen, in which the Spike protein keeps its structure, its folding, in
combination with the monoclonal antibody J08 that was developed by the MAD Lab group of
TLS directed by Professor Rino Rappuoli.

With our laboratory Toscana Life Sciences we have developed monoclonal drugs to treat
Covid infected people. Certainly having monoclonal drugs is important, but we must also
know who can benefit from them. To do this we need diagnostic tools, as we need to find out
who is infected with this virus. At Toscana Life Sciences we are lucky not only to be able to
do this, but also to have teams developing monoclonals and to know other teams like those
of DIESSE which specialize at realizing diagnostic tools. So as soon as we had monoclonals
it was a pleasure to share them with them.They have a very similar system to ours, a system
we are very proud of and which works well, because different teams specialized in doing
different things collaborate to solve problems.

We got to the idea of matching the use of monoclonal antibodies destined to developing
countries with an easy-to-use and affordable diagnostic tool reserved for them. DIESSE was
pleased to be engaged in this project and now we have finally managed to conceive a
prototype that should be out soon. It will be used for the clinical trials of the first monoclonal
antibody against covid in developing countries. Hopefully we will be able to commercialize it
soon.

The number of new cases is constantly increasing, especially with the spread of the new
variant Omicron that escapes the antibodies activated by vaccines. We therefore expect
more hospitalizations and more deaths. We will have to set up new pharmacological
resources, including new therapies with immunomodulators, whose use is closely integrated
with the monitoring of the new “Super” marker. That is why DIESSE is collaborating with
VIROGATES, for the dosage of the marker on its Chorus immunometric tool, opening more
than 6000 platforms worldwide for the measurement of this analyte. This partnership is

the patients who really need it.

Giuditta Pisani graduated in Linguistic Mediation at School for Interpreters and Translators Carlo Bo in Florence in 2012. She has excellent language skills in English, French and German, as well as in Russian and Spanish, which she is currently studying.
Her work experience as a receptionist in various 4 and 5 * L hotels in Italy and abroad has honed her customer satisfaction and problem solving skills, excellent spoken and written communication in several languages (Italian, English, French and German) and team spirit. She is very precise too, attentive to details and has a strong sense of responsibility.
In order to use her linguistic knowledge in other fields, she is currently working as an export office employee in a company that sells metal accessories for leather goods in Scandicci (Florence). She's in constant contact with English, French and German customers.
