Operacija mitralne stenoze kod koje kirurg izravno prstom ili nožićem (→ Dogliottiev valvulotom), nataknutim na prst, kida i reže, a time i proširuje suženo kruto ušće. Komisurotomija se izvodi na zatvorenu srcu i bez zaustavljanja srčanog rada. Prst se uvede u lijevi atrij kroz otvor u lijevoj aurikuli. Prethodno se oko otvora učini cirkularni šav (šav duhanske vrećice) koji se u vrijeme komisurotomije stegne oko prsta da se spriječi krvarenje iz atrija. Pošto se prst izvadi, šav se čvrsto stegne i sveže.
